To book your appointment, simply follow these steps:
STEP 1. Fill out the Appointment Request (below)
Canadian and US residences are welcome. Fill out the form below or call/text 204-471-8771.
You will then be contacted directly by a clinician to set your appointment date and time. All appointments are in central standard time.
STEP 2. Intake Forms
Once your appointment date and time is confirmed, you will be directed to fill out intake forms (by each person attending) prior to the first session.
STEP 3. Session Location and Parking (sessions are being held onsite, by phone, or by zoom)
- The office is located 566 Corydon Ave.
- Street parking is on Corydon Ave. and surrounding side streets
- For public transportation, take bus number 18 with stops within a block of the building
- The building is accessible with a ramp located on the east side of the building
- If your appointment is by Zoom, you will be emailed a Zoom link the day of your session
- If your appointment is by Phone, the clinician will call you at your scheduled time
STEP 4. Payments
You will be provided payment instructions by your clinician. E-transfer (preferred), credit cards or cash are accepted. For US residents, you will be invoiced via PayPal prior to the first session.
